Hazard: The recalled swing sets did not come with assembly directions for the swing seat harness. The swing seat harness assembly needs to be completed by the consumer. The swing seats can detach if the harness is not assembled properly, which could result in fall and injury during use.

Hazard: The written instructions incorrectly identify the proper location of the spring washer. An incorrectly installed spring washer can put undue stress on bolts, causing them to loosen, which poses a fall hazard to consumers.

Hazard: Stamina modified the warning/assembly instructions in the owner's manuals to emphasize that the elliptical pedal shafts must be securely tightened to the cranks. If not securely tightened, the pedal shafts could become loose from the cranks, which could result in serious injury to the user and/or damage to the product.

Hazard: The drain covers in the tubs can entangle a bather's hair in the openings, causing the bather's head to be held under water, which can result in drowning.

Hazard: A consumer's hand can become caught between the cylindrical counter weight and the frame of the fitness machine, posing a crushing hazard that can result in lacerations and finger amputation.

Hazard: An overinflated fitness ball can unexpectedly burst while in use, causing the user to fall to the floor.

Hazard: The end cap assembly on the rear of certain A-5® markers can break and eject during use, posing a risk of injury to the operator.

Hazard: Diving hoses may have been made without crimps, which can allow gas to leak or water to enter into the re-breather unit of the scuba diving equipment, posing a drowning hazard to the user.

Hazard: The ends of the bow's limbs can unexpectedly break during use and send fragments of the bow in the direction of the user or bystanders, posing a risk of injury.

Hazard: The binding plates could crack or break, causing the skier to lose control or fall and suffer injuries.

Hazard: Surface paints on the golf balls can contain excess levels of lead.

Hazard: The elastic harness system attaching the cleats to footwear can detach, posing a fall hazard to consumers.

Hazard: The heel housing of the bindings can crack, causing the binding to release unexpectedly. This can cause the skier to lose control or fall and suffer injuries.

Hazard: The helmet's chinstrap can unexpectedly disengage while in use. If this happens, the helmet can fall off, posing a head and neck injury risk to consumers.

Hazard: Children can fall or land on these dive sticks in shallow water and suffer impalement injuries. Eye and other facial injuries are also possible.

Hazard: The treadmills can speed up unexpectedly while in use due to a malfunction with the lower control board, posing a fall hazard to consumers.

Hazard: These regulators can be missing the diaphragm cover, which can cause the diaphragm to become displaced during a dive, allowing water to enter the scuba regulator. This poses a drowning hazard to divers.

Hazard: The fixed knot flexible openings in the soccer goal net can pose a head and neck entrapment or strangulation hazard to young children.

Hazard: Over-tightening of the DIN retainer by a technician during installation can result in the retainer breaking under pressure, a rapid escape of air from the scuba cylinder, and the regulator detaching from the scuba cylinder. This poses a drowning hazard to divers.

Hazard: The t-nuts that attach the buckles to the straps on the snowboard bindings can detach, posing a fall hazard to consumers.

Hazard: The alloy cranks on the exercise cycles can break during use, posing a laceration and abrasion hazard to users.

Hazard: The remote line adapter can burst when over tightened, posing serious impact and laceration hazards to consumers.

Hazard: The hip restraints on the cars can detach at the base, posing a fall and injury risk to consumers.

Hazard: Surface paint on the helmets contains excessive levels of lead, violating the federal lead paint standard.

Hazard: The cable that links the toe strap to the binding could break, posing a fall hazard to snowboarders.

Hazard: The hot tub's spa control can overheat, posing a fire hazard.

Hazard: The harnesses could fail during use, resulting in a hunter falling from the tree stand and suffering serious injuries or death.

Hazard: An internal component that seals air between the high pressure first-stage and the intermediate pressure second-stage can fail. This can result in uncontrolled flow of air to the diver and pose a risk of serious injury or death.

Hazard: The cranks that connect the foot platforms to the rear wheel of the machine can break during exercise, posing a fall hazard to consumers.

Hazard: The ankle clamp mechanism can release unexpectedly, posing a fall hazard to consumers.
