Hazard: If a child accidentally falls underneath the Unity Teeter Tunnel, the end of the seesaw plank can land on them, posing a risk of serious injury and a crush hazard.

Hazard: The clevis bearing on the swing set can wear, causing the swing to detach and the user to fall.

Hazard: The weld between the side arch support and the top rail can fail. If that happens, the swings can collapse, causing children on the swings to fall and be injured by the falling top rail.

Hazard: The welding of the slide bed to the slide sidewall can crack and separate and children's fingers can get caught in the space, posing a risk of amputation.

Hazard: The swings connectors, which attach the top bar to the end supports, can crack and the top bar can break off and hit children on the swings.
